There are several convenient ways to establish your personal Patient Portal account right now:

  • Provide the required information during your hospital stay. You will receive an informational pamphlet within your discharge packet. Complete the enrollment by following the instructions in the email that you will receive from Union Hospital.
  • Go to the Medical Records department in person and ask to enroll in Patient Portal.

What if I want to enroll in the Union Hospital Patient Portal at a later date?

There are several convenient ways to establish your personal Patient Portal account at a later date:

  • Provide your email address at the time of registration and they can supply you with your Medical Record number that you will need to enroll.
  • Go to the Medical Records department to enroll.

What information do I need to enroll in the Union Hospital Patient Portal?

To enroll in the Patient Portal we need to have your e-mail address on record. If you’ve not provided that on a recent hospital visit, you will need to provide it to Medical Records. You will also need to know the last four numbers of your Social Security number.

Do I have to provide my email address?

Yes, an email address is required for setting up your Patient Portal account. If you do not have an email address, you can create one for free through Gmail, Yahoo, Hotmail, or any other email service provider. Why do you need my email address?

It is through that email address that you will receive notification to log on to your secure Patient Portal account.

How will I know my Union Hospital Patient Portal enrollment was successful?

Once you have completed the enrollment paperwork, you will receive an email from the hospital. The message will contain a temporary user ID and password. Please use the temporary user ID, password and link provided to log in to Patient Portal. Please note that the enrollment email invitation is only active for 14 days from receipt.

  • Spouse: You should visit the Medical Records department to complete the Patient Portal Proxy form to allow access to your spouse. A copy of your ID will be required.
  • Durable Power of Attorney for Healthcare: The person who has been approved by you, Durable Power of Attorney for Healthcare may visit the Medical Records department to complete the Patient Portal Proxy form. A copy of ID will be required.
  • Minor: Parents may have access to minor patient records (0-17) by contacting the Medical Records department for Patient Portal Proxy form and present a copy of your ID.
  • Deceased patient: Patient portal information will no longer be accessible. Copy of the health record may be obtained by visiting the Medical Records department and presenting legal documents allowing authority.

My child turned 18 and I am no longer able to access his/her Union Hospital Patient Portal account. How do I reeestablish access?

You and your child will need to visit the medical records department together and complete the Patient Portal proxy access form.

I no longer wish to have proxy access to another patient's Union Hospital Patient Portal account. How do I turn it off?

If you have the medical identification number of the patient for whom you are the proxy, staff here at the Medical Records department can turn off your proxy access to the Patient Portal account.

I no longer wish to have access to Union Hospital Patient Portal. How can I close my account?

Please provide your state-issued photo ID, and staff at the Medical Records department can disable your Patient Portal account.

Does every Union Hospital patient automatically have a Patient Portal?

No. Though we strongly encourage you to sign up for Patient Portal so that you may benefit from its many convenient uses, a Patient Portal account will only be established for you at your specific request.

Is there a monthly charge for Union Hospital Patient Portal?

There is no charge. Patient Portal is offered complimentary to Union Hospital patients.

Your test results and reports are both available by clicking on the Health Record button on the portal home page. Once you click on the Health Record button you will be taken to the Health Record page where you will see a button on the right labeled Results where you will have access to your Laboratory Results and a button labeled Reports where you will have access to general reports. Laboratory results are available 36 hours after they are resulted.
